What if the whole incident happens when the ball is out of play? Player A will also receive 1x YC and 1x RC?

Once play is stopped for whatever reason THAT will be the restart, it will not change The additional misconduct occurring after the stoppage? IF determined by the official as red card VC then that red card COULD, in fact SHOULD, be shown immediately. As getting this trouble maker off the FOP is likely your biggest concern.
You still would record the USB from earlier in the match report. Plus you should inform the player and coach of that fact, there is a caution already on the books that is also being recorded
If you are concerned that the order should be a yellow card for the earlier USB and then the red card for the further VC. You might have time to do so given you are using the yellow to show to both players. Especially if you are in the process already and - then - the EXTRA VC occurs. I could see a quick finger point at, (you 15 red and you 12 blue) , are both cautioned shown the yellow card. They are likely near to one another and then the red card shown ONLY to to the player due to be sent off and his team down a player . A DIRECT red is a greater impacting sanction than a double yellow. We are assuming these two are not already on a caution
My reasoning is this, if two players are engaged it behooves us to get there quickly . If brandishing a yellow card, carried on the way over, it is likely already observable, so a quick shout out to the two adversaries, (15 red 12 blue) might stop the VC follow through or possibly the retaliation deterioration prospects! I reach back pocket for the red card the moment I see the violent contact, even while shouting, "Seriously I am right here!" . Cheers

View Referee Richard Dawson profileAnswer provided by Referee Joe McHugh Hi Petr The restart is easy in that one. The restart is the reason the ball was out of play so misconduct while the ball is out of play does not change the restart.
As to the sequencing of cards it is a judgement call for the referee to make based on the circumstances. If the referee has already shown a caution to a player and that player commits violent conduct then the red card is also shown. If the caution is not shown and a violent conduct situation happens my advice is to show the red card and have the player removed immediately. The totality of the situation can be covered in the report and it is important that the player and his team do not think it is a two caution red card rather than a straight red card which has a much stronger sanction.
